Eject has no effect,on or off or asleep. The manual eject button has no effect and the paper clip method has no effect.
Is this normal? Do I have a deformed card? It looks straight.
This doesn't conform to the user manual at all.
Is it safe to just leave it in all the time? Since it protrudes slightly past the end of the case it would seem vulnerable to damage.

I've rarely had a problem with cards not ejecting, either by drag-to-trash, command-Y, or the eject button. The few times it has happened, the paper clip has worked.

Your card cage could be damaged. Pulling out the card without using the button, the drag-to-trash, or the paper clip, can (and may in the past) cause damage to the cage.

There should be no problem leaving the card in all the time. You'll drain the battery a little faster, but that's about it.
