You're absolutely right; the little patch has to be applied to the installer as well as the app. You has'ta follow those patch directions meticulously, or else no tunes. Are you launching the iTunes installer, or the app itself? The only setup screens I remember are in the installer. IF it is the installer, I would say that the patch did not work correctly, and try it again. IF it is the app itself, I would suggest you look in the "Documents" folder on your HD and make sure there is a iTunes folder in there.
