2)      Threshold's are:  Any emergent on-line work greater than 1 mrem.
For daily work we roll up the monthly totals and if department dose
deviation is greater than 10% we generate Issue Report.  If they exceed
estimate for two consecutive months the Departments are assigned actions
to get their dose back on track.  During outage we do 50% and 80% WIPs
(if dose is off tract Issue Reports are generated).   We also write
Issue Reports for any lessons learned during Alara post job reviews,
pre-outage brainstorming sessions, or any improvement opportunities
identified during the outage.

I was wondering how other facilities document actual versus estimated
dose received.

 

1.       Do you write a Condition report for dose received above and
below the estimate?

2.       If so, what is the % difference that you use?

3.       Do you write them for discrepancies for both on-line and outage
dose?

4.       If you don't write a CR, do you document them some other way?
