Palo Verde provides the following response: 

Yes, Palo Verde requires a review by a supervisor or designated technicians of 
each radiation and contamination survey. 

Radiation and Contamination are quality assurance records with a lifetime 
retention requirement (lifetime of the plant). This is defined in ANSI N45.2.9 
- 1974, Requirements for Collection, Storage, and Maintenance of Quality 
Assurance Records for Nuclear Power Plants.

Section 3.2.1 describes the generation of records. This section describes the 
signature requirements. "The applicable quality assurance records shall be 
considered valid only if stamped, initialed, signed, or otherwise authenticated 
and dated by authorized personnel. These records may be either the original or 
a reproduced copy."

ANI Information Bulletin 80-1A rev 6 describes what should be on a survey:
*       Location
*       Date
*       Time
*       Survey information
*       Technician signature
*       Instrument
*       Reference to RWP
*       Other information which may be important

A supervisor review or second party verification is not strictly required. 
However surveys are required for liability purposes and to comply with 
regulations. Therefore, a review to verify the adequacy of the survey is 
prudent.
