electrical cords on vacuums?  Not to the cord but to prevent opening the vacuum 
/ HEPA we do




2. Does the procedure cover the use of locking device? Yes


3. Industry guidance recommends annual filter integrity testing or testing 
following opening the vacuum is this performed?  Is this conducted on a yearly 
basis or each time a vacuum is changed out?  Both, yearly and upon filter 
change out


4. Is it sufficient enough to consider the purchase of DOP tested hepa filters 
with certificate of conformance as testing to meet INPO guidelines? If so how 
was that handled by a white paper?  No, we DOP test all before we put in 
service as we have had new units fail


5. Vendor or manufacture to purchase testing unit to meet standards? No, we DOP 
test all before we put in service as we have had new units fail


6. Can I receive a copy of the procedure that governs vacuum control program 
and testing of units? Yes
