1. Yes, GEM-5

a. Yes

b. Yes

c. Workers inside the PA must successfully pass through a GEM-5 inside
the Security Access Control Building/PA exit. There is no passable route
around the GEM-5s w/o alarming them and security officers are stationed next to
GEM-5s, and attentive to the (very rare) alarms.

* RP does not have to be notified when a worker drops off their TLD (in
a designated drop box) before they exit the PA.

* For a worker terminating and already outside the PA (Training,
Engineering, off-site vendors...) their last PA exit (through GEM-5) is used as
their passive monitoring count.

* RP does provide a contact number for immediate notification if Form-5
is desired or address change is required.
2. Yes. Workers are required to contact RP prior to departure for other site.
After discussing travel particulars with the worker, we will either restrict
access and hold the TLD in Dosimetry until their near-term return, or terminate
their monitoring and reinstate it when they return if we suspect they'll be
monitored during their travels.

Benchmarking other Utilities:

1) Do you use a passive monitor similar to GEM5/PM7 when issuing and
terminating TLD/OSL use for personnel RCA Access?

a. Do you use a passive monitor for Entry(Issuing TLD/OSL)? Yes /No

b. Do you use a passive monitor for Exit (permanently leaving site and
removal of TLD/OSL use)? Yes/No

c. Is there any process or controls put in place to ensure notification
when they leave permanently when a passive monitor is used (if so, what is the
method used)?

2) Do you remove RCA access (contactors or Utility personnel) at your
facility when performing short term duties at other Nuclear facilities (e.g.,
Benchmarking, Assessments, outage support,) or short term medical leave,
military duty?

We have contractors (e.g., engineering contract personnel working out of
Dallas) that periodically come on-site).
