This map is very generic, hospital is one block. What everyone is looking
form is how information travels, who has access to, information with in an
organization.
For example: A patient comes to the hospital admitting desk and is
admitted, where does all the information go (hard copy, electronic,
interfaces to a variety of systems, ...). Then where does that information
then go (to still more Information systems, inside / outside the hospital,
ambulance services, treating docs, family doc, Billing, accounting,
photocopies of reports, faxes ...).
What is so hard to get hands around is who all touches the paper
information, electronic information, and the ever popular oral
communication.

Yoo-hoo!! Kristi?!! Where are you!! Yoo-hoo!!
Someone (Tanya Lang) other than Kristi or Cynthia already responded. No
need to redirect requests to anybody. I am reproducing that email,
verbatim, below (sorry Tanya, I seem to be one of the few who saw it):

There is much interest in a "map flow of PHI" and there is a wonderful tool
you can all access at:
www.ahima.org <http://www.ahima.org>
click on: "HIM Resources"
click on: "models and plans"
click on: "flow of patient health information inside and outside the
healthcare industry"
Have fun!

NOT sure if this PHI (patient health informaiton) is not what people are
looking for, as PHI is also (or actually) Protected Health Information,
which is mainly the personal information identifying the person.

Who is this "Kristi"? No "Kristi" has written into any WEDI listserve
that I've seen offering a PHI Map or map flow or Workflow or whatever.
This "Kristi" must be an urban-legend. It all started on Tuesday when
Cynthia Wise, apparently writing a private e-mail to some Kristi,
posted to the Privacy listserve seemingly by accident. Please forward
all requests to Kristi through Cynthia.
Does this prove my point that we should be more careful with e-mail?
