Not so much as covered to death but died a death.
The question was prompted recently by the (overpriced?) Americans and their proposed guidelines.

I usually charge a capture fee then my time for post production but even so, I often feel as though I'm working at a loss.
Personally I would love some guidelines or code of practice to work with, so I know I'm being fair to myself and my clients.
