I see this on many projects, and I'm not a fan either. But generally, it seems that a bot marks it stale, and then someone who cares (doesn't need privs) comments "hey @bot, not stale", but really any comment.
The other side of the coin is that issues and PRs can linger forever, not getting rebased, and for many, nobody cares. So this is a heavy-handed clumsy way of pruning those, vs a more labor-intensive way of manual review. I've done that manual review with maintainer hat on, for unison and for rtl_433, and it's a lot of work and a lot of edge cases.
