the mke2fs problem still points to HW, it could be bad
IDE-if but right now your prime suspect is the memory. 
Did you take out the faulty chip? Test the memory
very thoroughly to rule out problems.. I will email
you a memory tester tomorow. Until then 'e2fsck -f ..'
is a good indicator of trouble.

not easy to rule out.. there are many possibilities to 
get ports or interrupt conflicts. I have also had a flaky
io card and the symptoms were similar to yours. Otherwise 
I am using 2 ISA/IDE cards + 1 ethernet card and don't 
have any problems for more than a year so it certainly 
can work stable.

Mine is roughly 2 GB at the moment, I will rather
work to update the installation procedure somehow..
it has to be done anyway.
