I have thought for some time (and mentioned it to TT at one of the Paris shows) that formatting disks Under SMSQ/E is unreliable. At one point it used to hang the system (as demonstrated to TT at the same show)
I used to keep a standard QL and set of DD drives just in order to be able to format DD disks. Using 'FORMAT FLP1_*D' seemed to work better and Disk Mate 5 seemed to get it right most of the time but standard formatting has varied in reliability over the various versions of SMSQ/E. In particular the system seemed to lose the ability to recognise a DD disk as that and tried to format it as an HD one which often killed the disk.
