HI!!

First observe the log of Openldap by starting slapd in debug mode
slapd -d5&.
SEE!!If qmail is contacting ldap server or not.
If not means the ldap server is not bineg contacted means u will have to
see qmail is patched properly.
If LDAP server is contacted & it doesnot find the entry means u will have
to check the schema.
