It doesn't really apply to qmail-ldap, but I use a customized
change_ldappass with qmail-ldap and courier-imap.  I use the gnu
extension shadow md5 crypted format (ie $1$saltsalt$cryptedpassword) so
I have to set courier-imap to not rebind.  I use openldap and it seems
like it should be able to bind with that format of password, but it
doesn't want to for whatever reason, of course, I'm a few versions back.

If anybody is interested in seeing the modified change_ldappass, I did
submit it to the squirrelmail project:

It is a patch against version 1.0 of the plugin.

I also threw together a HOWTO on the courier/qmail/ldap thing (though it
is dated).  I don't think much has changed with regard to CourierImap:
