I have already tried this looking at my ldap logs with debug level 256. When I look in my ldap log, I see these types of lines:

Nov 21 13:08:11 tau slapd[13332]: conn=2003 op=1 SRCH base="ou=people, dc=protocolusa, dc=com" scope=2 filter="(&(objectClass=qmailUser)(uid=bclark))"

To me, this looks exactly as it should.

I don't think I have "catchall" configured. Is this a default? If it is not, then I don't have it.

My other question was why would qmail-ldap be delivering mail to an account with the accountStatus attribute set to "disabled"? According to the QLDAPINSTALL document, it should work like this, and bounce the messages going to that account.

LDAP_ISACTIVE (default: "accountStatus")

The status of a user account.
Values: active (no restrictions), nopop (only mail delivery but no pop access), disabled (bounce incoming messages)
deleted (bounce incoming messages and mark for deletion, see LDAP_PURGE)
Default: no accountStatus is equal to active.
