Documentation for sqwebmail with qmail-ldap

This is actually a followup question from a post that was in the archive.

Hi. If noone else is going to, I'd be happy to write up
what I did to get sqwebmail (and courier-imap) running
with my qmail-ldap setup... but I'm not clustering
or anything.

Does anyone have any documentation as to setting up sqwebmail with qmail-ldap successfully?

Can someone post their successful run file using tcpserver to set up a service to run?
