In a purely virtual user setup does qmail-lspawn need to be root? All of 
my mail and maildirs are owned by qmailv.  Is there a way I could 
reconfigure my qmail-lspawn to not run as root?

Correct,  I use qmail-scanner to handle virus scanning and spam assassin 
tagging.  My spam assassin is setup to handle per user 
whitelist/blacklists which are stored in a MySQL database.
