I usually just go in and remove the maildirsize file and that resolves the problem.

I've experimented some problem with qmail's quota counting. Occasionally some user receives the qmail QUOTAGUARD (threshold is 70%) when their Maildir is nearly empty. Besides maildisize files sometimes isn't updated when the user receives a new mail.
