I want Q-Mail to scan all the email, but I want it to run as the user receiving the email.� The way we currently have it set up is that all of our users are in SQL, some have a Spam level of 100, which is off, while other have 5,7,10 depending on the level of filtering.

IMHO that this is not possible (yet) as you want it.
If you refer to the FAQ :
"How do I make Qmail-Scanner only scan mail for some local users?. You don't. Qmail-Scanner is a server-based product. If you want per-user control, then you'll really have to look for something else. "
But I am setting up a per domain system. Don't know if I'll be using qmail-scanner for that or amavis + spamassassin alone. With SQL-based spamassassin you can easily deploy per domain rules - but you'll need another way the system works (maildropfilter oder procmail) - I think.
