when you ran 'qmail-scanner-queue.pl -g', did you get any output? Try (as root) 'setuidgid qmaild /var/qmail/bin/qmail-scanner-queue.pl -g'

and you should see output like:
  perlscanner: generate new DB file from
  /var/spool/qmailscan/quarantine-attachments.txt
  perlscanner: total of xx entries.
