Dear Brett,
I also try usr /var/qmail/bin/dot-forward in .qmail, still don't work.
ls -l /var/qmail/bin/dot-forward:
-rwxr-xr-x    1 root     root        29604 Aug  9 18:34
/var/qmail/bin/dot-forward*

what ownership should it be? ......
