mailing list creation issues

I've recently set up qmailadmin on my personal box, to help me manage
a few domains that I host for friends.

I have ezmlm-idx installed, and successfully compiled qmailadmin with
ezlmlmidx support.

Unfortunately, mailing list creation doesn't seem to work -- after
filling out and submitting the form, I am led to a blank screen with
the following url:

Something obviously isn't getting properly submitted, but I can't seem
to find any error output anywhere: not in apache's errorlog, not in
syslog, not anywhere else.

Any notion of where I should be looking?
