Exactly which patch are you using?  I think there are a couple out
there, but I've heard of some working better than others.

So what does your /var/qmail/supervise/qmail-smtpd/run (or eqivalent on
your system) have for the password checker programme?  And did you have
to change any perms for vchkpw to work with it?

I'd really like to know exactly how you got yours working.  I've been
wanting to get this qmail/smtpauth/vpopmail/qmailadmin combo working for
quite some time.
