Ok, try the following:

Goto the Qmail config directory:
(for me)
cd /var/www/html/config

Run config:
perl conf.pl

Choose to number next to plugins (8 for me).

Uninstall the following plugins by entering the number next to it (you may
have to scroll up if you list is as long as mine).

quota_usage
addgraphics
info
newmail

and any other plugins you think may be affected.

Hit S to save.
Hit Q to quit.

Then go back in and reinstall (by choosing those numbers again).  See if
that helps.

I can see an upgrade (qtp-newmodel command) breaking some plugins, but I
have not noticed these particular pluggins being broken in my upgrades.
Hopefully this easy fix works.
