Thank you both for the comments.

In my case, it turned out really to be a memory issue, and raising the
softlimit and _fully_ restarting qmail took care of it.

It seems to me that these three causes/solutions all belong in the
Troubleshooting section of the wiki. Should I create a wiki login and add
them, or would someone more familiar with qmailtoaster prefer to do it?

(Incidentally, I noticed some minor wiki spam-vandalism on the
Troubleshooting page, which probably needs cleaning up at the same time).
