I don't see your max_user_connects listed.

Albeit Postfix here is my dovecot -n  You should see the max_connections like highlighted.

You wouldn't have needed the qmailctl commands, but that's ok.
Please post the results of
# dovecot -n
so we can be sure your settings are correct. That command lists all 
non-default configuration values.
