Can you turn on SIMSCAN_DEBUG in either your
/var/qmail/supervise/smtp/run (export SIMSCAN_DEBUG=3) file or in
/etc/tcprules.d/tcp.smtp (SIMSCAN_DEBUG="3").
Try moving the databases /var/lib/clamav and doing a freshclam, maybe
there's a corruption.
You can try my simscan with debugging here (64bit only...for now) :

This new simscan will actually tell you WHY you're getting a qq soft reject.
