Ah, but qooxdoo will be breeding a whole new generation of HTML 
know-nothings like me. :)

Meanwhile, Werner's patch works great except the <noscript> warning I 
had in there that JS is turned off does not appear (if I turn JS off to 
test). Instead I get my fancy new "Loading..." animated gif.

I tried putting the loading code inside a <script> tag but then it does 
not appear under normal operation.

So I think there are two things the standard generated index.html could 
do: check that JS is turned on and put up a spinner by default. Those 
are both the way good UI apps should behave. And it would be relatively 
easier for folks to take things out of index.html than to figure out 
what to put in.

Right now I am abandoning the warning to keep the spinner working.
