I fixed this issue - SSL debug revealed it was permissions on the keys - which is odd because the keys were owned by the smtpd user that Apache::Qpsmtpd is running as, which also has read permissions to the files. I had to also add group read permissions to get this to work. Not sure why those permissions would be needed.
