However, they will probably tell you to fill that form and have your doctor sign it.  Most likely, your doctor is simply being lazy and not getting around to signing it.  (Unless he told you to your face that he will not sign it, and if that is the case find out why he will not sign it) first, physically go to your doctor's office and stay there until he signs it for you.  The secretaries may say he's busy, so you may want to try to set up a five-minute appointment in advance explaining why you are coming in.  If you just show up plans a campout for a while but don't give up, the squeaky wheel gets oil.
 
Finally after all that if your doctor still refuses to sign it for some strange reason, let him know that he's not the only game in town and simply go to a different doctor.  Also, sometimes on those forms they will accept verification from a therapist, an RN, or other officially titled individual such as medical assistance caseworker.  If you are on medical assistance they also must determine that you are disable and maybe a copy of this determination will be sufficient.

Contact a Federal Student Aid Ombudsman through the US Dept of Education.  Their job is to give you information and help with student loan problems.
You may have to find another doctor to sign the forms.

Does anyone know anything about getting defaulted student loans reduced?  I cannot afford the payments because I took out the loan prior to my disability.  My income from SSDI is not enough to make the loan payment.  I was one of fourteen students accepted into a new Honors programs at Indiana University this year, and now I can't even go to college.  Voc rehab cannot help until I resolve this loan matter.  I sent a form (the form that verifies I am disabled so that my loan can be reduced) from the financial aid office, to my doctor three times to verify that I am disabled, and they will not sign it.  I am a quad resulting from a T1 spinal cord injury.  How is that not a disability?  I should be responsible for my debt, but I cannot afford it at this time.  I desperately want to go to school.  Does anyone know what I could do?
