Same wound on my ishial, caused by shearing,i.e., my mattress is made up with 
air pockets and the bed has multiple movement settings. If I bring my head up 
without raising my feet first it causes a "shear" problem between my skin and 
skeleton. Picture your skin as a sack that. Houses your skeleton.

Never had any breakdown until the VA gave me the bed. Also, I have a ROHO 
Contro Select cushion that has my ishial suspended over a hole so it provides 
pressure relief in that. Area.

Meds Iam using:
Santyl to disenffect the wound covered by  Mepilex change when necessary.
Once odorless I use Aquacel in the wound to close up. 
This works best for me. Your wound specialist knows better.

The FIRST THING IS TO STOP WHATEVER IS. CAUSING THE PRESSURE.
