The error is in your note transform data.
I'm assuming you defined note transform number 7.
You wrote:

Did you notice something about that first "octave"? You've only got 8 notes
before you stepped down.
There are 128 MIDI notes and that is not a complete number of octaves, so
you tried mapping MIDI notes C-1 to B-1 onto C9 to B9, but the MIDI notes
only go up to G9, thus you are four notes short, hence the four semitone
difference you're hearing.
So, this is what I recommend:
As it is very unlikely you will be using MIDI notes C-1 to B-1 or MIDI notes
C9 to G9, just leave them unchanged, and make your transformation use
complete octaves in the range C0 (MIDI note 12) to B8 (MIDI note 119).
So your line should look like:
xfrm7=0,1,2,3,4,5,6,7,8,9,10,11,12,
108,109,...,119,
96,97,...,107
...,
12,13,14,...,22,23,
120,121,122,...,127
