Using the all notes off feature helps, but if that doesn't work, there's a much easier way to do it in VSt Host. Instead of restarting it, you can use the midi panic option, which has never failed for me. Go into VST Host, and I believe midi panic is in the engine menu. As for why the problem is occuring in the first place... maybe give your midi interface a look, as others have said. Also, if you have separate midi in and midi out, try disabling or unplugging midi out. If that works, then it is a midi setup issue and probably not a cable issue. Though if it's as simple as that, it would've caused deadly midi feedback a long time ago, if left unchecked, so I think that may not be the issue. Since I've never had this problem, I don't know if that's a worthwhile line of thought, but it's something that crossed my somewhat inexperienced mind of midi setup.
