I think your problem may be the ip address you've chosen.  Pay special
attention to this quote from portforward.com on their static ip guide.  

"The ip address you choose should be very similar to the router's ip
address. Only the last number of the ip address should be different. If
the router's ip address is 192.168.1.1, I might choose 192.168.1.10. The
ip address you choose should end with a number between 1 and 254, and
should not be the same as the router's ip address. Every device that
connects to your network needs to have it's own ip address."  

That quote is from their static ip guide.  It's very important that you
setup a static ip address also because if your ip address is changing
from being assigned by dhcp, your port forwarding rules won't always
work.  So if you haven't done so go here and follow the guide for your
operating system. http://www.portforward.com/networking/staticip.htm

The other thing I noticed is you need to uncheck "specify public ip
address" what I meant was to the right of "Networked computer/device:"
you should hit the drop down arrow where you have "squeezebox" selected
and select "Specify address."  (Sorry if I was confusing about that part
in my original post.) After that put in your new static ip address that
you have chosen.

Your port forwarding rules look good to me though.  So after you do the
above steps check them again with PFPortChecker and hopefully they'll be
open!

On more thing, I see in your original post your diagnostics say 
"- mysqueezebox.com registration [blank]:"
I checked the diagnostics menu on my radio and mine says 
"- mysqueezebox.com registration Registered"
So maybe make sure you've put your mysqueezebox login info into your
radio if you haven't.  I put mine in right when I first turned on my
radio but there must be somewhere to put that info in later. I'm not
sure where that is though.
