Here's how ...
01. EDIT ALL FROM SYS_FORMS3 ORDER BY SYS_FORM_NAME
02. Highlight the one you don't need based on SYS_FORM_MOD_TS and
SYS_FORM_VERSION
03. Press [F9] key to delete the row
