Vacuum the concrete floor, Use a set of pry bars to lift the edge of the case 
or a pic (flat edge) can be inserted under the edge of the case and you can 
step on the pointed end of the pick to assist in getting it up enough to insert 
a 1/2” pipe. Conduit works well too, I have different sizes of pipe, once you 
get the first pipe  centered under the case you can rock it to get one edge 
higher enabling you to use a 3/4 piece of conduit and so on. On the dirt floor 
use planks NOT plywood or flooring again depending on the height of the 
threshold use the various pipe diameters along w/ the center pivot point to go 
over the threshold. the same thing once you get it outside. 
Safety glasses, rope thru the case to aid in pulling and only hold your pipes 
w/ your finger tips. You will be surprised how easy things will be. I will 
usually have 3-4 pieces of conduit of various sizes.
