I use it, but last time I tried doing anything "fancy" I noticed it choked on subdirectories, so it can be a bit of an organizational nightmare.

Still does - not on each launch, but rather each time you view a plugin's documentation (i.e. the temp files are removed as soon as you close the help viewer).

format!? Must there be a plugin included or can I just make a Help only
plugin?

Yup, last time I tried that, it didn't work either. You might be able to get away with using empty stub libraries but the IDE probably expects more before "accepting" the plugin.
