They are.   They're paid by the page, and get no royalties, for many
books, for example Que, Sams, etc.

They do.  The editors do a lot.   They make arbitrary text actually
readable...


Personally, I can't see how a publisher that pays by the page and
doesn't pay royalties can expect a quality job from its authors, but
that is the state of (the majority by sales of) the publishing
industry.

There are notable exceptions -- Addison-Wesley, Prentice-Hall, and
O'Reilly come to mind.   Not that I know much about their processes,
just their products.
