We've just been running through this one, actually.

One thing to look at is the /etc/xinetd.d/tftp file.  Make sure that
the server_args line reads:

        server_args             = -s /tftpboot

And make sure the /tftpboot directory permissions are at least
drwxr-xr-x

That last x was missing on mine, and and looks to be what was causing
all of my problems.
