When I see issues like album art not appearing, I find that, without fail, I have a misplaced or mislabelled track somewhere other than where it should be.
Ie, there is a whole copy of 'Decade' by Neil Young, which has the cover.jpg and thumb.jpg as it should. But it doesn't show up in Browse Artwork. The reason: a mistagged track on a CSNY album (sometimes I'm not very careful with musicbrainz...) in a different directory.
