I've had a weird one cause multiple albums. My files are named track# - title.mp3 . Well I had a few double albums show up because a file was in use in another program when mp3tag renamed them. All that was wrong was it showed as track#title.mpt no spaces and no dash. Everything else was the same and once I fixed this they all went back to one album. I didn't think that track name could effect album structure.
