Thanks Kyle for your suggestions. I followed them, added the line extension=mssql.so and restarted the server and was still getting the same error. I then did as you suggested, checked the error_log and there are some errors. Here's a copy of a section of the error_log file with the last service restart:

Also, Anna is correct in mentioning you'll want to use mssql_ functions and not the odbc_ functions. While ODBC should work in theory I've never actually seen
it work between php and mssql.
