Here are some realistic operating accessories that I have seen:

Operating crossing gates with flashing red lights and sound.

Flashing red lights on crossbucks with sound.

Moving wig-wags with sound. (Which always stop in the down position)

Rotary coal loaders.

Moving gates on a chain link fence.

Operating steam shovels (same as Michael Eldridge described)

Operating cable system for loading logs onto flat cars.  In HO.

Operating water tower spouts with sound.

Operating coaling tower chutes with sound.

Operating forklift that pushes large boxes onto flat cars.

Operating semaphores.  (Two blades per mast in HO)

Operating drawbridges with slowly moving ships passing beneath.

Operating hump yards with car retarders.

And all that is just off the top of my head.  Just think of what the list would 
look like if I actually thought about it for ten minutes.

Some operating accessories are very realistic and authentic.  Some are not.  
Just like engines and track.

There is nothing inherently wrong with accessories that operate.  It is the 
degree of realism that is of interest.
