Okay I think I'm able to reproduce it now. If a check is using both the primary and secondary SMTP alerts, and you delete one of them, they both disappear, and your left with some sort of phantom blank entry that you can highlight and delete. If there are other alerts besides the SMTP alerts, they seem to disappear as well when one of the SMTP alerts is deleted.
