1] the template file
2] output from SA to a web server (I use IIS on the same box as SA and I output as an asp file so I can pick up my server name)
3] locate image files in a subdirectory called images (didn't include my company image, just the icons I use for status on the page)
4] locate the js file in the same directory as 2 above.
 
I'm not a web programmer by any means, so I'm sure this can be cleaned up.

I haven't yet implemented the web-based monitoring on my implementation as my HTML skills are a little rusty, what would I need to do to make use of your template?

Here's a template I've developed myself.  it gives a quick heads-up, then you can actually click on the numbers (down, maint, etc.) and go to the next anchor point.  also the included js displays all the fields I know about when I hover of the icons.  built as an asp page to pick up my monitoring servers name basically.
 
the page is bulky, but it gives me the info I need (except for dependency stuff - of all events, not just events that are dependent on events that are down.
