Just for anyone who wants to use NT Authentication for SQL Server checks, this is how we have SA setup.
 
The service is running under a domain user account.
We have ODBC DSN's setup to use Windows Security for each of the databases we want to check.
Then we use the ODBC query add-on to execute a stored procedure in the database.
The domain user account has very limited access and can only execute that procedure.

It's a bit of a drag having to setup the DSN's, but at least we can now check two servers that only accept Windows Authenticated logins.
