I wonder if the fact that my ISP wants to charge an extra $10 for every "client" that 
"shares" an Internet connection has anything to do with creating this problem.

Michael,

My guess would be that your upstream provider is using a caching device
(NetCache) that is returning this error to your proxy (Sambar) which in
turn returns it to your client.
