I tried your suggestion to use satchmos default form.html template and I get the same behavior.

Before you try a fresh install of satchmo, if you can convert back to the default template (form.html i think?) so that you can know its an issue, i think it would be a lot quicker and easier for you than trying to upgrade to satchmo.     Have you edited the html or the javascript?  Maybe something was
    changed on accident.

    If you have edited the templates, try reverting back to the
    default template (copy your template else where) and see if you
    still have the issue.
