Why don't you use the texts recommended by the license itself
(in the section "How to Apply These Terms to Your New Programs")?

Did you check all files in your tarball?
I think we should set a deadline: you've already have been working on this
issue for a long time; if it isn't fixed within a reasonable period, it must
mean that you really can't follow Savannah hosting requirements.
