Rod, Cannot say why this volume was not attached to your instance, since it is terminated now.
As of EBS volume deletion. Most likely, you accidentally hit 'Delete' instead of 'Detach'. Delete operation does not ask for confirmation, volume delete request is sent immediately. We will add a confirmation step to this operation. We have checked the source code and can say that Detach has nothing to do with volume deletion.
