No, this also happened to me, even though they appeared according to the 
settings, to be setup to have VoiceOver to speak through both.

I simply turned off and back on VoiceOver for each channel, and then after that 
VoiceOver came out of both channels for me.

When I was using iOS 10 on my old 6S, I did see this problem occur
once. I thought it was my earbuds, so I switched them out with another
pair and the same thing happened. Ultimately, when I either restarted
VoiceOver or rebooted my phone, the sound returned to both sides. I
don't recall which I tried or which worked though. It seemed to be a
VoiceOver issue rather than an issue with hearing accessibility.

I hear VO on both earpods with 6S Plus on iOS 10. The only
  difference I noticed iPhone doesn't unlock anymore when connecting
  it.
Did you happen to mess with hearing accessibility settings? there
  are some balance settings.

I have seen this a few times on my 6s plus. Simply restarting VoiceOver has 
fixed it for me.

I have not noticed that. I just put on a pair of headphones plugging them 
directly into the headphone jack of my 6s, and voiceover is coming out of both 
channels just as it always does.

I've noticed this: audio only comes out of left-channel. I constantly had
to go to VoiceOVer audio settings and re-select the right-channel (even
though it was selected).
